Once your customer account is validated, you will be instructed to read and sign our E-Commerce Terms and Conditions User Agreement. Please read through it carefully and check if the information you entered is correct.
Once complete, you must print and sign both sections of the Agreement and fax it to (847)-578-8034 or mail it to: ARLINGTON, Attn: IT Dept, 1616 Lakeside Drive, Waukegan, IL 60085.
